New UIL Form Required for Benbrook High School Athletes.

Benbrook families preparing for fall sports have one more form to complete before student-athletes can practice or compete.

Beginning July 1, the UIL added a mandatory PAPF Eligibility Screening Questionnaire to Fort Worth ISD’s Rank One system for high school athletes. Even families who completed their athletic forms and physicals before July 1 must log back in and submit the new requirement.

Returning students in grades 10 through 12 who attended Benbrook Middle-High School for the full previous school year will generally answer “No” to the first eligibility question. Incoming freshmen and students new to Fort Worth ISD may need additional review, so families should answer carefully and contact the athletic staff with questions.

Students must also complete all remaining UIL and FWISD forms and submit a current physical and medical history before participating.
